Fill out an online form for a COVID Vaccine Appointment found HERE, or also found on the homepage of CSKThealth.org

Once the form is submitted, a Tribal Health staff member will call to schedule your appointment for vaccination.

Moderna vaccine is for 18 years and older, Pfizer vaccine is for 16 years and older.
