Attention- Community Medicaid Outreach Events

ALL TRIBAL HEALTH RECIPIENTS:

We have seen a drastic decline in our Medicaid/HMK/CHIP numbers!  Join us at one of our upcoming events in your area to get you + your family BACK ON MEDICAID!

Join us while our Healthcare Resource Team assists you & your family in completing/submitting your application for Medicaid, HMK, or CHIP.

There will be: INCENTIVES, CATERED MEAL, DOOR PRIZES, & ACTIVITIES

*Have your name entered into a raffle for BIG prizes upon completion of application (this means that ALL necessary documentation included)! Prizes include: 43″ Roku Smart TV; Adult’s Bicycle w/ helmet; Children’s Bicycle w/ helmet

*Bring these documents with you to the event: Valid ID; Income Verification (last 30 days); Newborns- birth certificate will be required
